Complete this form to assist FiberLight to properly configure the customer Dedicated Internet Access (DIA) service and improve the service activation process.  If using static routing, only complete the IPv4 / IPv6 portions of the form.  If using BGP, please fill out the entire form.  New forms are required each time there is a change in the customer IP address configuration of the DIA service. 

FiberLight IP Address Policy

For IPv4, requests greater than the original supplied /30 or /29 will require completion of this IP Justification Form.  FiberLight will allocate up to a /25 based on demonstrated need and compliance with ARIN‑aligned utilization requirements.  For customers requesting greater than a /25, FiberLight recommends the customer self-source IPv4 space through the ARIN or IPv4 brokers. 

Customers MUST use 60% of the IP addresses requested on the day of installation, with a one-week grace period to cover equipment problems. Customer must use 80% of IP space after 90 days of installation, or FiberLight has the liberty to reclaim FiberLight provided IPv4 space.  Customers must use 90% of IP space before requesting additional IP space from FiberLight.
